    Job Description
    Job Title:  Community Integration Counselor (CIC)
    Status:  Per Diem
    Purpose:  To assist waiver participants who are experiencing significant problems managing the emotional responses inherent to adjusting to a significant physical or cognitive disability while living in the community.  
    Position Snapshot:  The CIC assists in the recognition, identification, and ongoing management of limitations presented by disabilities that create emotional discord among the individual and/or the primary caregiver(s). 
     
    Essential Functions:
    • Provide CIC as established by NHTD and TBI manuals.
    • Establish a professional relationship with participants and ensure that their self-directed services are provided.
    • Ensure proper review and submission all required documentation for billing.
    • Attend provider meetings as directed.
    • Performs ongoing recruitment via developing relationships to "market" AIM Independent Living Center as a waiver service provider.
    • Contributes to team effort by accomplishing related results as needed.
    • Maintain confidentiality and wield discretion on sensitive matters regarding participants.
    • Pursue professional development opportunities in conjunction with performance evaluation goals as well as participate in agency quality improvement measures.
    • Maintain agency compliance as presented in the employee handbook.
    • All other duties as assigned and requested not listed above.
    Education and Experience:
    • Licensed Psychiatrist – Licensed by the NYS Education Dept.
    • Licensed Psychologist – Licensed by the NYS Education Dept.
    • Licensed Clinical Social Worker (LCSW) or Licensed Master of Social Worker (LMSW).
    • Doctorate Degree in Psychology.
    • Mental Health Practitioner – Licensed by the NYS Education Dept.
    • Certified Rehabilitation Counselor – Certified by the Commission on Rehabilitation Counselor Certification, and
    o   Must have, at a minimum, two (2) years of experience providing adjustment related counseling to individuals with Traumatic Brain Injury and their families and/or seniors with physical and/or cognitive disabilities and their families. 
    Qualifications:

    • Strong interpersonal, communication, organization, writing and computer skills.
    • Ability to effectively interact and work with individuals from diverse backgrounds.
    • Attention to detail.
    • Ability to multi-task and work effectively in both a team and individual setting.
    • Flexibility and ability to manage a complex workload in various environments.
    • Ability to travel to multiple locations.
    • Bi-lingual proficiency preferred.
    • Valid driver’s license with satisfactory driving record and reliable transportation.
    Physical and Environmental Requirements:
    ·         Sitting at the computer.
    ·         Extensive reading and researching relevant topics.
